Part 3 begins exactly where Part 2 left us.

I decide to continue the EKG a little longer.

I ask her to raise her legs to about 45 degrees and hold them there, so I can see how her heart responds when her body has to work a little. She obeys. She tries to keep her legs up, slightly tense, while the EKG continues to follow every beat. Her expression says everything: focused, nervous, compliant.

Then I begin checking her pulses carefully. Femoral, behind the knees, ankles... I work through them methodically, comparing what I feel, making sure everything is as it should be. This is the kind of detail I never want to rush. A proper exam needs hands, patience and attention.

After the EKG, I ask Jessica to remove the sticky pads from her chest and stay where she is. She does it carefully, still not entirely sure if we are finished. We are not.

I apply a little more conductive gel and take out the ultrasound probe. With Jessica lying supine, I begin listening to her heart again through the probe, following the sounds of her valves and inner cardiac movement. The sound has a different texture from the stethoscope, and I take my time with it.

Then comes one of my favorite moments in the whole exam.

The camera moves close to Jessica’s neck, and her pulse is clearly visible. Not every patient gives us this. With Jessica, it is beautifully obvious: the small movement at the side of her neck, her heart showing itself from the outside, beating while she lies there exposed and still.

I then turn her onto her left side, into the left lateral decubitus position, and continue with the ultrasound probe there. I listen again, checking how the heart sounds change in this position, following each area carefully. Jessica remains patient and cooperative, letting the exam continue exactly as I need it to.

When I finish, I give her paper tissue to clean the gel from her chest. She sits up on the examination bed, still looking a little uncertain, waiting for my final words.

I tell her that we are done and give her clothes back. But while she begins to dress, I decide to check her BP one last time. Her reading was a little elevated at the beginning of Part 1, and that can easily happen when a patient first arrives at the office feeling nervous. I want to see if it has settled now that the exam is nearly over.

While she puts her tights, bra and top back on, I explain my findings from the full cardiac examination. Jessica listens quietly. That fiery red hair, that beautiful body, that polite nervousness... she really is a 10/10 patient from beginning to end.

For me, this is where the examination feels complete. The EKG findings - raised-leg response, pulse checks, cardiac ultrasound observations, visible neck pulse, and final BP comparison... all come together to give a fuller picture of Jessica’s cardiovascular assessment.

The left lateral decubitus position. Her heart now shifts closer to the chest wall. Sounds that are subtle in other positions become clearer here, more present, easier to isolate. It is where a careful examiner goes when he wants to hear everything.

And I want to hear everything.

I work through her cardiac areas again, slowly, repositioning the stethoscope with care, listening for the murmur, checking how it behaves, how it sits in the rhythm of her heartbeat. Jessica is curled slightly on her side, exposed and patient, her red hair falling softly as she holds herself still for me.

Then it’s time to check her breathing. I perform a thorough lung exam, first with Jessica sitting upright as I place my stethoscope all over her chest. She takes continuous deep breaths through her mouth as I listen. I reach behind her and auscultate her back, as the deep breathing continues. This is all captured in a fascinating split-screen view which I just love. I tell her to lie down flat again, and continue listening to her lungs all over her chest a second time.

Then it is time for the ECG.

I ask her to sit up and remove her pantyhose. She does so carefully, a little self-consciously, the way Jessica does everything. Once they are off, I attach the limb electrodes: the metal clamp leads, one on each wrist, one on each ankle, cool and precise against her skin.

Then the chest leads. Six sticky electrodes, placed one by one across her bare chest in the correct anatomical positions (V1 through V6), each one pressed gently into place with the care that the procedure deserves. I take my time here too. There is no reason to rush.

Watch her eyes during this part! She has been calm through most of the exam, but something about the ECG changes her expression. Maybe it is the knowledge that the machine is about to read her heart directly, every beat recorded, nothing hidden. Maybe it is that it's the first ever ECG she's having and she comments that it's "a bit scary"... Whatever it is, a quiet nervousness comes over her.

The machine starts and the beeps begin, steady at first, then a with a suspicion of respiratory arrhythmia. Jessica lies there, bare chested and perfectly still and connected. Beep-beep-beep-beep...

I want to listen again, slowly, carefully, with no rush.

Before reaching her heart, I perform the preliminary checks: pulses, glands in her neck, mouth examination with the tongue depressor, eyes with the penlight, temperature under her arm, and BP. Jessica answers softly and follows every instruction. She is exactly the kind of patient I love examining: compliant, attentive, a little shy, but trusting the process.

Then it is time.

I tell her I need to examine her heart properly, and for that, her top and bra must come off. She is embarrassed at first. She removes her top, hesitates, and tries to keep herself covered for a few extra seconds. But she understands what I need. She complies.

Now her chest is fully exposed, and the real examination begins.

I take my time over every cardiac area. No rushed placements, no fake gestures, no empty roleplay. Just a proper heart exam on a beautiful, cooperative woman sitting quietly in front of me. I listen for rhythm, tone, timing, subtle extra sounds, anything that might explain the murmur I heard during the house call.

For cardiophiles, this is the kind of moment that matters: the stethoscope moving slowly across her bare chest, her body staying still, her eyes uncertain but obedient, her heart being studied with genuine attention.

Then I palpate her chest to locate the PMI and feel the movement of her heart beneath my fingers.

After that, I ask her to lie down. In the supine position, I continue the cardiac auscultation, going back over her exposed chest, listening again from a different position, checking how the sounds change while she lies flat, watching her reactions as she tries to stay calm.

At the end of Part 1, I turn her onto her left side for the left lateral decubitus position. That is where the next part will continue, with even more focused cardiac listening and a full, detailed lung examination.

I tell her I need to examine her heart properly, and for that, her top and bra must come off. She is embarrassed at first. She removes her top, hesitates, and tries to keep herself covered for a few extra seconds. But she understands what I need. She complies.

Now her chest is fully exposed, and the real examination begins.

I take my time over every cardiac area. No rushed placements, no fake gestures, no empty roleplay. Just a proper heart exam on a beautiful, cooperative woman sitting quietly in front of me. I listen for rhythm, tone, timing, subtle extra sounds, anything that might explain the murmur I heard during the house call.

For cardiophiles, this is the kind of moment that matters: the stethoscope moving slowly across her bare chest, her body staying still, her eyes uncertain but obedient, her heart being studied with genuine attention.

Then I palpate her chest to locate the PMI and feel the movement of her heart beneath my fingers.

After that, I ask her to lie down. In the supine position, I continue the cardiac auscultation, going back over her exposed chest, listening again from a different position, checking how the sounds change while she lies flat, watching her reactions as she tries to stay calm.

At the end of Part 1, I turn her onto her left side for the left lateral decubitus position. That is where the next part will continue, with even more focused cardiac listening and a full, detailed lung examination.
