It’s late at night and Sarah Heizel, a nun for the town’s church, finishes her duties before going to bed. That is until Alexa Flexy, a woman with a worried look on her face, enters the church. She wishes to make a confession urgently but Sarah tells her that none of the priests are awake. Alexa pleads for Sarah to take her confession instead.

Despite trying to tell Alexa that it is not for her to perform, witnessing the frightened woman beg for confession pushes Sarah to invite her into the confession booth. With fear in her voice, Alexa describes what she believes to be a demon manifesting inside of her, taking control of her mind and body.

Believing what Alexa is describing to be nothing more than sinful thoughts, Sarah tries to advise Alexa that she must expose this ‘persona’ and challenge it. Alexa closes her eyes and takes a deep breath. What emerges from within Alexa is something far more supernatural than mere bad thoughts. The entity possesses Alexa’s body, manipulating her flesh and corrupting the church, keeping Sarah frozen in place. Sarah is now at the mercy of something from the depths of hell itself, eager to unleash its sinful impulses.

Detective Lia Lin enters a derelict asylum with a flashlight and investigates the building, observing the events that have happened to the camera’s owner, Sarah Heizel. She encounters a woman in a medical gown facing the corner, who appears to be Sarah. Lia pulls out an image of Sarah, smiling innocently in a standard photo, and believes her to be Sarah. As Lia approaches her, she is restrained by Sarah, a ghostly woman who appears from behind her.

Sarah Heizel, a young woman, explores the interior of an abandoned asylum, taking photographs of the rusted medical tools and old sheets. She hears a whisper and takes a photo of the room, which reveals two ghostly women in medical gowns, Alice Peachy and Dolly X. The ghosts, dressed in pale faces, dark markings, and dirty medical gowns, grab Leah, making her suck their dicks. Sarah’s expression changes from horrified to aroused, and they ejaculate all over her face.

Alice kneels in front of Sarah, with Dolly kneeling behind her. Sarah strips away her pants and underwear, licks Dolly’s cock, and Alice slides her dick inside her pussy, making her moan through her clogged mouth. The ghosts moan to the sexual pleasure of using Sarah’s body. The photographer’s moans are reduced to retching and gargling, making Sarah roll her eyes back. Dolly gets overwhelmed by Sarah’s constant sucking, falling back onto her ass. Sarah lunges forward, entering a downward dog pose as she continues sucking on the ghost’s cock. Alice leans forward, drilling into Sarah’s pussy, stopping herself from ejaculating.
